Personalized concierge medical care for one member.
Annual membership for two household members: $2,500 for the first member plus $1,500 for the additional member.
We believe care should extend to the whole family. The individual annual membership is $2,500, and each additional family member in the same household is offered a reduced rate of $1,500 per year.
Membership can be paid annually, semiannually, quarterly, or monthly. We are confident our model delivers the care you deserve, and we can tailor the payment schedule for you. Call the office for details.
IASIS Boutique Health is different from many concierge practices because we also offer pre-operative evaluations for non-members.
If a non-member is seen by the doctor and decides on the same day to become a member of the IASIS family, the visit fee is waived.

The individual membership is $2,500 annually. Each additional family member in the same household is $1,500 annually. A two-member Family Plan is therefore $4,000 annually.
IASIS allows membership payments to be arranged annually, semiannually, quarterly, or monthly. Call the office for payment details. The membership fee renews annually regardless of the payment schedule selected.
No additional charges apply to the services specifically listed in the membership plan, including unlimited office visits, the listed annual labs and point-of-care tests, EKGs, unlimited televisits, and facilitation of an initial consultant appointment.
Concierge membership is not a replacement for health insurance. Patients may still need insurance for hospital care, emergency services, specialists, procedures, medications, imaging, and services not included in the membership.
Yes. Non-member pre-operative evaluations are $500 and are offered depending on availability and at the physician’s discretion. If the patient joins the practice on the same day as the evaluation, IASIS waives the visit fee.
